Special Olympics Fitness Evaluation: Lifestyle Survey
Description: A survey created for athletes to determine improvements in health and health related behaviors. This survey also includes questions to understand athletes’ satisfaction with fitness programs.
Target audience: Use by Program staff, coaches, and fitness professionals to assess athletes and Unified Partners.
Length: The survey takes 5-8 minutes to complete.
Measures:
Physical activity (i.e., frequency of moderate physical activity, hours of sedentary behavior, muscular strength activities per week)
Nutrition (i.e., fruit and vegetable consumption, hydration)
General health and fitness status
Gains in physical activity and nutrition knowledge
Gains in physical activity and nutrition confidence
Gains in social support for physical activity and nutrition
Fitness program satisfaction (e.g., did the athlete enjoy the program?)
What’s a ‘Qualtrics Report’?
A report of your Lifestyle Survey that will update automatically as data is collected.
You’ll be able to see who responded to the survey pre and post.
This will be shared through a secure cloud platform. Only your SO Program data will be shown in the report.
Give Qualtrics link to athletes before or on your first day of fitness programming.
Send athletes an email with the link before your first session or bring tablets or phones to the first day of the program to have athletes complete the survey.
Did you miss anyone? Check your ‘Qualtrics Report’ provided by SOI to determine if you’re missing athletes. Send a reminder email to complete the survey or remind them at your fitness program.

Send athletes an email with the link after your last session. Do this within a week of your last day of programming. Or bring tablets or phones to the last day of the program to have athletes complete the survey.

Results & Reports: The Research and Evaluation team will provide a link to your ‘Qualtrics Report.’ If you want detailed analyses of your pre/post data, the Research and Evaluation team will create a summary report for you!
