- Cover Letter: Programs are encouraged to use a cover letter to provide a brief introduction and submission instructions to athletes and their families. This letter can be customized for your Program's needs.

ATHLETE REGISTRATION FORM
The registration release forms include:
- Registration Form: This form asks for contact and health history information.
- Waivers Releases and Policies: This form goes over risks, use of likenesses, emergency medical care, consent for health program participation, personal information, and other important details.

Programs are encouraged to consult local legal counsel to ensure the forms are compliant in their jurisdictions. If you need to make substantive changes to the Release Forms, please contact the SOI Legal Department for approval.
ATHLETE MEDICAL FORMS
The medical form was updated in 2017 when the current Registration Form was created to reduce duplication across the two forms. The revised medical form collects information in two parts:
- The Health History is the first two pages and is completed by the athlete, parent or guardian prior to the exam. The health history is important for two reasons: (a) to have medical information on hand during training and competition, in case of a medical emergency; and (b) to ensure that the physician is informed of the athlete's health history when performing the exam.
- The Medical Exam is the third page and is completed by a licensed health professional who is qualified to conduct physical exams and prescribe medications. The medical exam page indicates if the athlete has been cleared by a health professional to participate in Special Olympics sports. If an athlete requires further examination due to a concerning health issue before clearance can be determined, a referral form is available on the fourth page. Customization of this form is not permitted.
This is the recommended Medical Form for all Programs. However, the revised Athlete Medical Form is not yet required in all situations. Learn more about when to use the revised Athlete Medical Form by clicking HERE.
NOTE: At this time there is no requirement to enter data into the Games Management System (GMS), although it is encouraged. Additionally, as we transition to the new Healthy Athlete Software (HAS), there is no requirement that data be entered into HAS for MedFest.

SUPPLEMENTAL FORMS
Emergency Medical Care Refusal Form
The Emergency Medical Care Refusal Form (formerly called the Religious Objections Form) is rarely used and should only be completed if an athlete or legal guardian does not consent to emergency medical care on religious or other grounds and has marked a box under the Emergency Care provision on the Athlete Release Form.
It is recommended that Programs not include this form in general registration packets and instead make the form available upon request or by download online.
