Virtual Athlete Release Form (DOCX)

This optional form may be used for any individual with intellectual disabilities (and/or unified partner) who was NOT previously registered but is participating Special Olympics’ virtual/online fitness, sports, leadership and inclusion programming services.

General Waiver of Liability, Assumption of Risk, and Indemnity (US example) (DOCX)

This form was created especially for and at the request of U.S. Programs due to a shared corporate insurance program and their liability concerns. The primary intent of this form is for fundraising event participants. However, this is an example that other Regions may choose to use for their fundraising events as well. Programs must keep in mind that depending on the jurisdiction, the effect of signed waivers, of the waiver requirements themselves may differ. Consult with legal counsel locally.

Communicable Disease Waiver for Participants

For U.S. Programs only: All athletes, Unified partners, coaches and volunteers must submit this waiver to their respective Program prior to returning to in-person activities. Please read the accompanying memo for full details and frequently asked questions.
