Forms for Volunteers and Athletes

All Special Olympics volunteers and athletes need to fill out forms before taking part in activities. Your local program will have the forms you need.

For New Program Leaders

If you are a Program leader who needs to create or revise forms, get in touch with your Regional office's organizational development personnel.

Several Kinds of Forms

Most Special Olympics activities involve filling out a form of some kind. 

Forms provide Special Olympics with information needed to keep track of who's involved with different activities. Forms identify special needs that people have. Forms also take care of some basic legal requirements. 

Special Olympics Programs around the world use forms that appropriate for their location, so be sure to get in touch with your local Program to get the forms you need.

Athletes, Volunteers, Unified Partners

Special Olympics athletes fill out forms that give details of the athlete's medical history and current health. The medical forms need to be renewed every few years. Here is a guide to athlete registration forms for Programs.

 

Volunteers who work closely with our athletes on an ongoing basis, such as coaches and officials, fill out one kind of form. Those volunteers are considered Class A volunteers. Those who help out on occasion at events fill out another kind of form. 

 

People who don't have intellectual disabilities and who want to be on a Unified Sports team will fill out a Unified Sports partner form.
